Program Description

Our Adult Day Program provides support to older adults and seniors who have a cognitive impairment, such as Alzheimer’s and dementia, and are physically frail. This program allows caregivers to receive some respite during the day to rest, recharge and complete everyday tasks.

We provide a supportive environment with social, therapeutic and recreational activities, along with gentle physical and mental exercises to stimulate and engage the individuals attending our program.  Nutritional, well-balanced prepared meals are provided during the day including a morning and afternoon snack and full lunch. Transportation may also be available upon request for an additional fee (based on availability).

COVID-19 SERVICE UPDATE

Clients and their caregivers are expected to passively screen themselves prior to receiving services or attending programs and to immediately report the presence of symptoms or risks.  We ask all clients/caregivers to refrain from attending program if feeling unwell and advise us of any COVID-19 symptoms.

While we promote a “mask-friendly” environment, the use of masks is strongly encouraged during the provision of care and while in program spaces and vehicles if tolerated.

Eligibility

An eligibility assessment is required to attend this program.  Our Case Management services will guide you through this process and support any questions you may have.

Our programs are designed for older adults who are:

  • isolated and vulnerable with physical impairments;
  • socially isolated;
  • living with Alzheimer’s disease or other forms of dementia;
  • recovering from stroke.

Our day program operates from 10:00 am – 3:00 pm, Monday to Friday.

Saturday 10 AM-3 PM for Rosemount and Wesburn
