Service Description

Our H.E.A.R.T.S Caregiver Program provides professional support, educational resources, and networking opportunities for older adult caregivers who care for adults with developmental disabilities.

Using virtual tools, H.E.A.R.T.S. program staff support clients with:

  • future care planning;
  • access to developmental services;
  • caregiver wellness activities, and
  • regular workshops on topics of interest to caregivers.

Educational Resources: Supporting families in caregiving

Below are a series of educational modules and workshops specially developed for caregivers of persons with developmental disabilities. Educational modules were created in partnership with Humber College.

Module 2: Supporting families (di)stress reduction

Module 3:  Supporting families in caregiving – Increasing your resilience

Module 4: Supporting families in caregiving – Healthy aging accepting change

This program is offered in collaboration with Humber College, Community Living York Central, and other agencies that support adults with disabilities and their caregivers and is fully funded by the United Way Greater Toronto’s Allan Slaight Foundation.

Eligibility

Open to older adult caregivers (ages 55+) caring for adults (ages 18+) with developmental disabilities.

Fees

No cost. This program is funded by United Way Greater Toronto’s Allan Slaight Seniors Fund.
