The Case Manager works alongside the Director of Quality, Communications, and community Outreach with the day-to-day operations of the H.E.A.R.T.S. (Helping Empower and Recognize Tools for Seniors) Caregiver Program. The case manager participates in the development of programs and activities for clients, implementation of procedures, and best practices.
From May 12th to 16th (anticipated), the main entrance will be closed due to work related to the accessibility project (installing ramps and other related work). The front door entrance and the surrounding area will be inaccessible. Expect a few parking spaces closest to this work area to be inaccessible. Thank you for being so patient as we improve the accessibility of our building.
During this time, access to the building will be available through the side door of the Rosemount building.
