Community Hospice & Advanced Care Planning: An information session

Who will speak for you?
No-one knows what life has instore for us. That’s why we insure our cars and our homes. Advanced Care Planning is about ensuring your wishes guide the decisions made for you, if you ever become unable to make such decisions yourself. In this session you are invited to ask yourself what is important for you? Who do you want to speak for you if you are not able to? And to learn why these are important conversations to have with your loved ones now.
What is hospice care?
Many people have a misconception that hospice is a place you go to die. Hospice is so much more than last days or weeks. Hospice palliative care is an approach that aims to reduce suffering and improve quality of life for people who are living with a life-limiting illness and their families. In this session you will learn more about community hospice supports available to people with illness, their caregivers and the grief supports available for those who are bereaved.
