Take Charge of Your Health
Based on a Chronic Disease Self-Management Program (CDSMP) developed by the Stanford School of Medicine, our CDSMP provides education and tools to help you live healthier and better manage day-to-day tasks while living with chronic disease.
During this program you will learn how to:
- Manage symptoms, medications
- Lead a healthy lifestyle and manage stress
- Manage pain and fatigue
- Make healthy food choices
- Appropriate exercises
The program is FREE and participants meet for 2.5 hours a week over a 6-7 week period. Each participant receives a copy of the manual “Living A Healthy Life with Chronic Conditions” which is filled with proven tips, suggestions and concrete strategies.
Anyone living with or caregiving for a person with a chronic condition such as Heart Disease, Arthritis, Diabetes, Asthma, Bronchitis, Emphysema, Depression and/or Anxiety is welcome attend this program.
