ESS is involved in two province-wide initiatives to reduce Alternate Level of Care rates in hospitals.  With the funding and support from the government and community agencies, ESS has implemented the following Alternate Level of Care programs:

The Bridges of Care

This initiative provides short-term, around-the-clock personal care and support to individuals who are leaving the hospital and waiting for long-term care.  ESS is delivering this program with $323, 200 of funding from the Mississauga Halton Local Health Integration Network (LHIN).  With this funding, ESS has put into service 12 beds for this program.  Bridges of Care is part of the capacity identified to support Trillium Health Partners’ Capacity 99 Plan. 

Reintegration Care Models

This initiative provides short-term, around-the-clock personal care to individuals who are transitioning from hospital.  A supportive, home-like environment is provided to prepare individuals to return to a new home such as long-term care, a retirement home or other residence.  During their stay, they are prepared for reintegration by normalizing daily routines, engaging in social recreation, and promoting physical mobility.  This program is funded by the Toronto Central Local Health Integration Network (LHIN).  With $211, 200 funding, ESS has put into service 8 beds for this program.
